About GCP

GERIATRIC CARE PARTNERS, LLC was established in 2006, by Sherry Vogel, MSW, to assist elderly clients and their families in obtaining and maintaining the highest quality of life for our elderly. Our goal is to assess the medical, mental and psychosocial needs of the elderly client in order to provide advice and/or on-going geriatric care management.

An initial assessment by a Professional Geriatric Care Manager is completed to assist the elderly client or family in identifying problems and probably solutions. Follow-up geriatric care management can be provided by the Professional Geriatric Care Manager if the elderly client or family is unable to do these themselves.
